YOU MUST DOWNLOAD THE APPROPRIATE LAB FORM AND TAKE WITH YOU. YOU MUST FILL IN YOUR NAME, ADDRESS, PHONE, DATE OF BIRTH AND EMPLOYEE ID (IN PATIENT ID FIELD) PRIOR TO ENTERING THE PATIENT SERVICE CENTER.
